Photo/Video Release Form

I, the undersigned, hereby grant 15fifteen Media Club and its authorized representatives the irrevocable right to use photographs and/or video recordings of me and/or my child(ren) in all forms and media for promotional, educational, advertising, and any other lawful purposes.

I understand that I will not receive any compensation for the use of these images or recordings. I hereby release 15fifteen Media Club and its representatives from any and all claims related to the use of these materials. I confirm that I am the legal guardian of the child(ren) attending the event and am signing on their behalf.
